Description
The study intends to measure health-related quality of life (HRQoL) among ambulatory chronic HF patients who receive HF care in MOH specialist centres, and to explore the relationship between patient clinico-sociodemographic characteristics and HRQoL. This study is a multi-centre, cross-sectional questionnaire-based survey to be conducted between March 2023 to March 2023 (12 months) in participating Ministry of Health (MOH) hospitals where patients attend outpatient specialist clinics for HF care. Eligible subjects who consent to participation will be requested to answer a Patient Survey Form in a language they choose (English, Malay or Mandarin Chinese). The Patient Survey Form consists of questions related to socio-demographic characteristics, and two HRQoL instruments, i.e., EQ-5D-5L and AQoL-6D. Then, site investigator or data collector will then collect patient's clinical data from their clinical notes using Clinical Data Collection Form. All the information collected will be analysed to answer the research questions.

Inclusion & Exclusion Criteria
Inclusion Criteria
Adult patients (≥18 years old) who have a diagnosis of heart failure (HF) for at least 3 months at screening and receive outpatient care for HF in MOH specialist centre.|Patients who are able to understand sufficiently any of the languages used (Bahasa Malaysia, English, or Mandarin Chinese) in the health survey, and are competent to give their consent to participate in the health survey.|Patients who are unable to provide responses but are accompanied by a caregiver (proxy) who consent to participate in the study.
Exclusion Criteria
HF patients who are hospitalised at the time of recruitment.|Patients who have impaired cognitive function (e.g., dementia) or any physical or psychiatric disorder that may hinder completion of the survey.|Patients who have a diagnosis of active cancer or have received radio- and/or chemotherapy in the last two years.|Patients who have other end-stage non-cardiac diseases, including renal disease (stage 5 CKD or on dialysis), liver disease (Child-Pugh C), and/or lung disease such as advanced chronic obstructive pulmonary disease (COPD), including patients who are on palliative or hospice care for non-HF related conditions.|Patients who are unable to complete the health survey on their own, including those who cannot respond verbally, and are not accompanied by a caregiver who consents to the study.
