Malaysia Primary Health Care ABC: Mapping the evidence on the landscape of diabetes, hypertension, and dyslipidaemia management (MyABCMap)

General Information
In the Malaysian healthcare system, a comprehensive strategy has been devised, and significant resources have been allocated for effective management of T2DM, hypertension, and dyslipidaemia. The recent emphasis has been on implementing interventions at the primary care and community levels, accompanied by regular audits and the monitoring of quality indicators. However, optimal control of these three chronic conditions is yet to be achieved. This underscores the need for a more profound exploration into the root causes.
General objective:
As such, our study aims to synthesise the evidence on treatment and outcomes of patients with T2DM, hypertension, and dyslipidaemia at the primary health care level in Malaysia in order to identify areas for potential improvement to optimise care for these three chronic diseases.
Methods:
The objectives of this study will be answered in two stages using different study design: scoping review for stage 1 and qualitative research for stage 2.
In stage 1, a scoping review will be conducted to synthesise the evidence on treatment and outcomes of patients with T2DM, hypertension, and dyslipidaemia at the primary health care level in Malaysia. The scoping review will follow the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for scoping reviews (PRISMA-ScR) and the Joanna Briggs Institute (JBI) guidelines. Search will be conducted in PubMed, Embase, Scopus, and MyMedR using relevant keywords and medical subject headings terms. Grey literature will be included by searching the Ministry of Health reports. Search period is from inception till 22 February 2024. Inclusion criteria:
a) studies/programmes conducted among individuals with T2DM, hypertension or dyslipidaemia, or their healthcare providers
b) studies/programmes conducted in Malaysia
c) studies/programmes conducted at the primary health care level
The qualitative research in stage 2 aims to determine facilitators and barriers to sustainability and upscaling of an effective intervention/programme, and to identify potential actionable solutions.
- Study population: stakeholders who are experienced and directly or indirectly involved in the management of T2DM, hypertension or dyslipidaemia at the primary health care level: healthcare providers (Family Medicine Specialists, medical officers, nurses, allied health professionals), healthcare administrators at different levels (district/central), academicians, programme implementers or evaluators, patients and caretakers. Objective 4 will not involve patients and caretakers as this objective focuses on sustainability and upscaling. Objective 5 will include all stakeholder groups.

The estimated duration of the entire research is 12 months (from May 2024 to June 2025).
